Office Policies For New Patients

We encourage new patients to print and complete the New Patient Registration Form and the HIPAA Form prior to their first appointment.

If you do not have access to a printer, you will receive a copy at the office. However, it will save time if you can bring a signed copy with you.

Existing patients are not required to fill out the registration form.

Please remember to inform the staff of any changes to your records.
